Homeless Case Manager I - Posting #25305

Hourly Rate: $20.00

Position Summary:

Full-time Case Manager position available for our Homeless Program at our Tacoma-11th St Clinic. We are seeking a highly motivated, energetic, self-directed individual who is responsible for ensuring individuals who are homeless (including migrants) have access to quality health care. This includes primary health care (including preventive health and dental care), substance abuse (drugs and alcohol) treatment and counseling, and mental health services. Individual will make all efforts to reduce barriers to care which includes case management services such as assistance with entitlement applications, translations, referrals for shelter, employment and job training, among others. The person in this position must be willing to work with the homeless population.

This person must be a self-starter, team player, dependable and have excellent customer service skills. Must be organized and with demonstrated ability to identify, initiate and coordinate special projects of various sizes. Individual must be at ease with homeless and migrant populations as position requires outreach at shelters and/or migrant camps. Individual must be flexible with work schedule in order to best meet client needs (periodic weekends and/or weeknights required). 

Education, Skills and/or Experience Requirements:

  • Proficiency with Microsoft applications (specifically Word, Excel and PowerPoint).
  • Knowledgeable and sensitive about the needs of homeless and migrant populations.
  • Good organizational skills.
  • Knowledge of electronic medical records desired but not required.
  • At least two years of college level education and/or two years general social services experience.
  • Must have or obtain a CPR/BLS card within 90 days of employment
  • Flexibility with schedule: although typical schedule is Monday-Friday 8:00 am-5:00 pm, some evenings and/or weekend work will be required.
  • Well-developed communication skills and team player.
  • Bilingual in English and Spanish required (written and verbal).
